Lea Valley Health PCN invite you to our second patient engagement event, the theme for this event is Frailty Awareness.
Practices in the Lea Valley Health PCN are:
- Cromwell Medical Centre
- High Street Surgery
- Stanhope Surgery
- Stockwell Lodge Medical Centre
The event is being held to inform patients, family, carers and friends about the work we are doing to help patients with the condition that effects all of us eventually, Frailty.
There will be stalls for each practice, local charities and information stands. Patients will be able to speak to practice staff and other health professionals.
Patient Participation Groups (PPG) from each practice will also be there with guidance on how you can sign up and become a voice for the patient community.
